School Information
Residency Requirements
It is the policy of the Board of Education to deny applications for school attendance by non-resident students, except for students enrolled in a cooperative program with another district, who are exempt. Resident students enrolling in the district for the first time may be required to provide three documents from the following list as proof of residence:
- Title evidence, mortgage papers, lease agreement, or completed Student Registration and Proof of Residency Questionnaire and notarized Affidavit of Residence (1 only)
- Utility bill (1 only)
- Photo identification (may include driver's license, Illinois state identification card, etc.)
- Tax bill
- Current voter's registration card
- Loan payment book
- Home insurance policy
- Bank account
- Medical card
- Green card
The district may require periodic verification of residency as a means of validating compliance with the proof of residence requirement.

Health Information
All students enrolled in the Manteno school system must provide appropriate medical information. Necessary health record forms and information are available at the links below. *Forms cannot be submitted online. Please print them out to turn in.
