NOTICE OF PROCEDURAL SAFEGUARDS FOR PARENTS/GUARDIANS OF STUDENTS WITH DISABILITIES  This notice of your procedural safeguards must be made available to you only one time a year, except that a copy also must be given upon an initial request for evaluation, a receipt of the first written complaint or first due process complaint to ISBE, upon a disciplinary removal that constitutes a change in placement, or upon request.
