MEDICATIONS

 

OVER THE COUNTER MEDICATIONS

 

  • Any Over-the-Counter (OTC) medication, such as *Cough Drops*, Tums, and other OTC remedies, will only be administered when accompanied by the Over-the-Counter (OTC) Request for Administration of Medication at School Form. This form must be signed by the parent/guardian and the student's health care provider.
  • All Over the Counter Medication must be in the original packaging, with the students name, name of medication and dosage.
  • These form are located on this website by clicking on Medication Procedure to the left of the screen.

* Students are not administered cough drops in the Elementary or Early Childhood Center due to choking hazards.

 

 

PRESCRIPTION MEDICATION AND SELF ADMINISTERED MEDICATION

 

  • Forms for Prescription Medication and Self Administered Medication can be found on the website (manteno5.org). 
  • Click on the Health Services tab and then Forms.

 

*Students will not be given Prescription an/or Over the counter medications without a completed Medication Authorization form which has been signed by the parent/guardian and the health care provider.

 

School Year 2016/17-medication forms were updated according to IL School Law and reflect the most updated changes which were implemeted.

 

Medication pick-up day is Thursday, May 25, 2017. Reminder notices will be sent home with your students.
